ICSI: helping sperm and egg meet, precisely

Intracytoplasmic sperm injection (ICSI) is a specialised form of IVF in which a single healthy sperm is injected directly into each egg. It is often used when sperm count or movement is low, and is carried out in our lab by experienced embryologists.

What is ICSI?

One sperm, one egg, carefully matched

The first steps are the same as IVF: medicines help your ovaries develop several eggs, which are then collected. Instead of placing eggs and sperm together, the embryologist selects a single sperm and injects it into the centre of each mature egg using a very fine needle.

  • Helps when sperm can’t easily reach or enter the egg
  • Can be used with fresh, frozen or surgically retrieved sperm
  • Embryos are watched closely for 3–5 days
  • Extra good-quality embryos can be frozen

Who it’s for

When ICSI is usually recommended

Your doctor will suggest ICSI when it gives better chances than conventional IVF.

Low sperm count

When there are too few sperm for fertilisation to happen on its own.

Poor sperm movement

When sperm swim weakly or not in the right direction.

Abnormal sperm shape

When many sperm have a shape that makes fertilisation difficult.

Earlier fertilisation failure

When eggs did not fertilise well in a previous IVF cycle.

Frozen or retrieved sperm

When using frozen sperm or sperm collected surgically.

Few eggs collected

To make the most of every egg when only a few are available.

The ICSI process

What happens, step by step

ICSI follows the same path as IVF, with one difference in the lab. Your doctor will give you a clear calendar.

About 4–6 weeksfor one full cycle, from the start of medication to your pregnancy test
  1. 1

    Consultation & tests

    1–2 visits

Semen analysis, hormone tests and a scan help confirm whether ICSI is right for you.

  2. 2

    Ovarian stimulation

    About 8–12 days

Daily hormone injections help your ovaries develop several eggs, with regular scans to track them.

  3. 3

    Egg collection

    15–30 minutes

Eggs are collected under sedation with a fine, ultrasound-guided needle. Your partner gives a sample the same day.

  4. 4

    Sperm injection (ICSI)

    Same day

The embryologist selects healthy sperm and injects one into each mature egg.

  5. 5

    Embryo culture

    3–5 days

We check fertilisation the next day, then watch the embryos develop and choose the healthiest.

  6. 6

    Embryo transfer

    A few minutes

A soft, thin tube places the embryo in the uterus. It’s usually painless.

  7. 7

    Pregnancy test

    ~2 weeks later

A blood test confirms the result, and we plan the next steps with you.

Success factors

What affects your chances with ICSI

ICSI improves fertilisation when sperm is the main issue. Pregnancy still depends on several factors.

01

Age

Egg quality and number decline with age, especially after 35.

02

Egg quality

Only mature, healthy eggs can be injected and fertilised.

03

Sperm quality

Sperm DNA and shape still play a role in embryo development.

04

Embryo quality

How well embryos grow in the lab over the first days.

05

Uterine health

A healthy lining helps the embryo implant.

06

Lifestyle

Weight, smoking and alcohol affect both partners’ fertility.

Our honest approach: ICSI helps sperm and egg meet, but it can’t guarantee a pregnancy. After your tests, Dr. Rekha will talk you through your personal chances.

ICSI FAQs

Your ICSI questions, answered

Still unsure? Ask us on WhatsApp

What is the difference between ICSI and IVF?
In IVF, eggs and sperm are placed together so fertilisation happens on its own. In ICSI, a single sperm is injected directly into each egg. The rest of the cycle is the same.
Is ICSI more painful than IVF?
No. For you, the experience is the same as IVF. The extra step happens in the lab.
Can ICSI be used with very low sperm count?
Yes. ICSI needs only one healthy sperm per egg, so it can work even with very low counts. In some cases sperm can be retrieved surgically.
Does ICSI guarantee fertilisation?
No. It improves the chance of fertilisation, but not every injected egg will fertilise or grow into a healthy embryo.
Are babies born after ICSI healthy?
Most babies born after ICSI are healthy. Your doctor will explain any specific considerations for your situation.
Can extra embryos be frozen?
Yes. Good-quality embryos can be frozen and used in a later transfer.
